Here are the links to the Idaho High School Activities Association Physical and Interim Questionnaire Forms.
Please print out and return to the LHS office.
Freshmen and Juniors are require to turn in physicals, sophomores and seniors are required to turn in Interim forms. (Physical form is two pages).
